The American star in the roundel replaces 6 points in the dice Nermal is rolling. There are also dice sets with other WW2/Cold War emblems, both made by Battlefront Miniatures, Warlord Games, and aftermarket companies, as well as dice for Warhammer games (sci-fi and medieval) made by Games Workshop - just those have skulls engraved instead on 1 point.
