I don't know if I've asked this before (and I apologize if I have) but what do the designs on his shoulders mean?

They're the biohazard warning symbol. Waaaay back in the day, when HAZARDouszilla was known as HAZARDFreaK, she did quite a few drawings related to toxic waste and pollution. Cleaver actually used to look like a bizarre skully horse!
