This is the youma(?) Mizuumi no Youkai (Creature of the Lake) from Episode 41 of "Pretty Soldier Sailor Moon".
This monster was once a beautiful village girl from ago whose jealousy of a beautiful water sprite who won her boyfriend's heart, turned her into this thing.
The then-evil Prince Endymion released her to do his bidding, but she refused, attacked him, ran amok and terrorised Usagi Tsukino and her family!
After sailor Mars (rei Hino) discovered that the monster was NOT a youma (apparition) from the Dark Kingdom, Sailor Moon healed her and the spirit of the village girl was free.
